Class B Claimants – Distribution Packet Submission
Who Should Submit
Class B claimants with approved Proofs of Claim (POC) must submit both their
completed distribution packet and a completed W-9 form. Multiple submissions
for the same POC are not permitted.
If you have an approved Class B claim, you should have received a letter from
the Special Deputy Receiver (the “SDR Letter”).

Refer to your SDR Letter
Verify your packet type (BI, PD, or PH).

Complete all forms
Fill out the distribution packet and sign/notarize as required.
Complete and sign the IRS W-9.

Submit both documents
Upload them together as a single submission (up to two files), or
Mail them to the address provided in your SDR letter.
