Class B Claimants – Distribution Packet Submission

Who Should Submit

Class B claimants with approved Proofs of Claim (POC) must submit both their completed distribution packet and a completed W-9 form. Multiple submissions for the same POC are not permitted.

If you have an approved Class B claim, you should have received a letter from the Special Deputy Receiver (the “SDR Letter”).

✅ Submission Steps

  1. Refer to your SDR Letter
    Verify your packet type (BI, PD, or PH).
  2. Complete all forms
    Fill out the distribution packet and sign/notarize as required.
    Complete and sign the IRS W-9.
  3. Submit both documents
    Upload them together as a single submission (up to two files), or
    Mail them to the address provided in your SDR letter.
